Initial Comments:
An offsite follow-up survey conducted on July 22, 2021, found that Patriot Home Care, had corrected the deficiencies cited under 28 PA Code, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries. The deficiency was cited as a result of a re-licensure survey completed on May 10, 2021.
